I object to this "where no man has gone before" business. It's totally sexist, and wrong, because Picard said "where no one has gone before." 8)

sigh... you're right. Stupid Berman and Braga screwing up continuity. Of course it's "no one" since we know Lily was on the bridge of the ENT-E and would have seen that's it's "no one." She would have told Cochrane that he's sexist and made all of Trek since then into an alternate time line. Why can't they have any respect for what came before!!!!!

After finding a slightly clearer version and fighting crosseyedness, I think the top line of the fine print is "Starfleet Command: R. Berman B. Braga. X. Xxxxxx P. Lauritson X. Xxxxxx" (and so on - the rest is illegible)
I'll also add that there appears to be a helluvalot more crew names on this one than any previous plaque... 9 lines, with about ten names a line. Perhaps the whole rendering team at Foundation got on there? Either that or the First Company Grips and Catering Coordinators caught word of the plaque and refused to show up for work without getting assuraces that they'd be on there, too.
