USS Discovery -- "The Squire of Gothos" (TOS) -- This one's still debatable. One would suspect it wasn't intended to be a ship name, but Kirk's poor grammar causes it to only make sense if it is. I choose to include it. You may not.
USS Republic -- "Valiant" (DS9) -- Some people think this is the same as the Constitution-class ship. I doubt it.
USS Yamato (five of them) -- "Where Silence Has Lease" (TNG) -- If we accept the NCC-1305-E registry as valid (which I do), there must have been five other Yamatos to precede the one we saw.
I also have some questions about things that were on your list. You may have simply copied them from other sources, and other people would have better answers. Hopefully I'll get the answers from someone...
USS Cochrane, NCC-8000 -- I think I remember this coming up before. How exactly did we find this out? Was it visible in the episode, or did someone simply claim that that was what the ship said?
USS Dauntless -- More a comment than a question... This wasn't a real Starfleet ship.
USS Defiant -- Another comment... You can't seriously think that the ship was supposed to have said that in WYLB, can you? It was just laziness on the part of the VFX guys. It should be ignored.
USS Jenol*n -- Actually, maybe I have more comments than I have questions. Anyway, the correct spelling of the caves for which this ship was named is "Jenolan". Since no two onscreen sources agree on the spelling, I'd recommend going w/ that one.
Kai Winn -- Are you sure this was a "USS" ship? I hope not...
USS Kongo -- There really isn't any evidence that an Excelsior-class by this name was ever used. I may have just been an idea in Okuda's head that never made it any further.
USS Lexington, Excelsior-class -- This ship wasn't actually seen in TNG, and the line in DS9 about the other Lexington, IIRC, pretty much says that the TNG one has to be the same as the DS9 one.
Shiku Maru -- Was this ever established to be a "USS"? The Encyclopedias don't give it a prefix.
USS Spector -- Wasn't this just in the Fact Files? I thought the Akira in MiaB was either unlabelled, or still labelled "Thunderchild".
USS Voyager, NCC-73602 -- Is this assuming the monstrosity seen in the DS9TM is actually the Voyager preproduction model, and that it was actually seen in DS9 somewhere?
USS Yellowstone -- I don't see any reason to assume this exists in the normal timeline, but some people do.
USS Yorktown -- Why assume the one mentioned in "Flashback" is a new one?
USS Zandura -- As mentioned, this wasn't actually in the movie.
NCC-13471 -- I'm just lonst on this one. Weren't the up-close shots in WYLB that showed a Miranda or two just stock footage?
NCC-73916 -- I have NCC-73918. I seem to recall there being a debate over this one in the past. Was a definitive answer reached, or did everyone just go w/ their preference?
What should be on your list is the possibility of a pre-TOS era U.S.S. Dauntless with the registry of NX/NCC-01. The suffix of "A" on Arturis' Dauntless might imply that he was taking the name from an early Starfleet ship. I, however, choose to believe that the new form of registry was meant to indicate the new form of propulsion. But that's me.
That's a hell of a lot of work just to kill Voyager's crew.
"Computer: Design a Starfleet-like ship with more advanced drives. Paint logos on the hull. Pick a name and registry that's a logical choice for such a ship, given our access to Federation data banks, and edit this intercepted message as follows..."
I mean, if the Enterprise can whip together Moriarty based on an off-hand challenge ("design an opponent who can defeat Data"), how much further do you need to extrapolate?
Heck, maybe he just said, "Computer, I want to mess with Voyager's crew. Figure out a way."

3. Actually, I just added that hyphen accidentally. The actual registry was NCC-1305E without a dash before the E. I tend to think it was just a part of the registry number and not a sequence indicator. (Like the TAS ships.)
4. The U.S.S. Cochrane NCC-8000 is from the TNG Companion.
5. I am aware the Dauntless wasn't a real Starfleet ship. However, it may actually be based on a real ship. Plus, I'd just add it anyway to add because even though it was fake I like the Dauntless and think it deserve a place in the chart. (Sorry.)
6. Whether it was an error by the VFX people or not, the onscreen evidence clearly and repeatedly shows that the new Defiant is still NX-74205.
7. I agree that the 'a' spelling is probably the most accurate, but for now I'm listing the one given in both the Encyclopedias and the Fact Files.
8. I'm fairly sure that the U.S.S. Kai Winn was indeed on the ship readout in "Whispers" (DS9). Go figure.

Let's discuss the latter part. The argument for the third Defiant's registry is that since it is visible as NX-74205, the registry is valid. The counter-argument is that the VFX staff were rushed and didn't have the time to complete all changes.
I am not taking sides in this debate. However, I will point to several registry issues. Here they are:
U.S.S. Hood
This Excelsior Class starship is introduced in "Encounter at Farpoint". The underside of the saucer is seen as the ship pulls away from the U.S.S. Enterprise. The registry is that of the U.S.S. Excelsior-NX-2000. Later, in "Redemption, Part 2", the U.S.S. Hood is given an unique registry-NCC-42296. Then, finally, in "Tears of the Prophets" and according to insiders, the U.S.S. Hood is given the registry of the U.S.S. Lakota- NCC-42768. If we follow the argument illustrated above for the U.S.S. Defiant, then the U.S.S. Hood started life with registry- NX-2000, then had a change in registry-NCC-42296, and finally was given another registry-NCC-42768. This is seen for the most part on screen and can thus be ruled canonical. Or we could argue that the U.S.S. Hood had only one registry and that the other two registries were inadvertent.
S.S. K.E. Tsiolkovsky
This ship, from "The Naked Now", has one visible registry-NCC-638 (seen as the ship careens into the solar fragment). This is the registry of the U.S.S. Grissom, of ST III. On the other hand, there is the dedication plate which is seen and not readable in the episode. This has the registry of NCC-53911. Since the visible evidence is more noticeable, can we say the S.S. K.E. Tsiolkovsky had the Grissom's registry? Or is this another instance of inadvertence?
U.S.S. Prometheus
This ship has two registries-NX-59650 and NX-74913. This one has been the focal point of many controversies and I will simply point to this case.
Danube Class starships & U.S.S. Rio Grande
This is a more interesting case. The first known runabout in Star Trek history, the U.S.S. Rio Grande had the registry NCC-72452. This is seen when the ship is ascending to the outer hull of the station. AFter this introduction, every runabout, including the runabout destroyed in the TNG episode, carried this registry to "One Little Ship". In that episode, the U.S.S. Rubicon was seen with a registry-NCC-72936, followed by the U.S.S. Shenandoah with registry NCC-73024. Then we see again the U.S.S. Rio Grande and her sister ship, the U.S.S. Gander. Both ships carry the registry NCC-73024. Finally, one of the last appearances of the runabout had the ship carrying registry NCC-73918.
So, in effect, the visual evidence tells us that from 2368 to 2374, all runabouts with the possible exception of one had registry NCC-72452. Then in 2374, runabouts had two registries. A year later, the runabout had two registries with the last registry as NCC-73918.
I will leave you to figure this mess out.
My personal opinion-
The models of ships are symbolic of the ship and her mission. The registry and name are often less important than the visual message that is being conveyed.
U.S.S. Cochrane
In the first episode for this ship, as confirmed by the TNG Companion and StarTrek.com, the U.S.S. Cochrane is seen in "The Drumhead". Her registry is four digits long and could be NCC-8000 or NCC-9000, which could account for the ease that this ship is transformed into the U.S.S. Yosemite NCC-19002. Returning to the subject matter, this same ship is seen in "The Game" and "A Fistful of Datas", as the U.S.S. Biko. Then in "The Emissary", we see an Oberth Class starship docking with DS9. Her registry is NCC-19002, that of the U.S.S. Yosemite. (BTW, this could be argued as proof that the ship seen in "A Fistful of Datas" was stock footage. And the ship seen in "Realm of Fear" was a new reuse of the Oberth Class model.) According to the StarTrek.com site and the Encyclopedia, this is the U.S.S. Cochrane. Years later, we see this ship on a display in DS9.
So in summary, the U.S.S. Cochrane has the following registry history-
NCC-8000 or NCC-9000
"The Drumhead", "The Game"
NCC-19002
"Emissary"
NCC-59318
"In the Pale Moonlight"
The thing about the Defiant is that we see it multiple times, not just once, and clearly, so it doesn't really matter WHY it was onscreen. It was and so that's what we have to accept as the reg.
It's not 'S.S. K.E. Tsiolkovsky' it's U.S.S. Tsiolkovsky. Whatever the dedication plaque says, it's the name and prefix in the encyclopedia that are considered correct. The NCC-53911 is the correct reg.
The NX-59650 reg is correct for the Prommie because that's what was most visible (plastered all over the hull) in the ep. That's also what the Encyc says in the text of the entry. The other graphics are overruled by the text.
The latest theory is that there were two Oberth-class U.S.S. Cochranes. The NCC-8000 from "The Drumhead" and the NCC-59318 from "Emmisary" and "In the Pale Moonlight."

Besides, there are only a few cases where what he says even does contradict what's onscreen.
I'm sure we all know that Okuda sent an urgent memo to the Producers to say that the reference to 1305-E be ommitted, as it was not a canon registry. They unfortunately commissioned a second draft of the script, whereby the old 1305-E was inadvertently re-inserted.

As for the Prometheus registry. I prefer to, and use the NX-74913 one on my site. Whether it was barely seen or not, it was still there. The problem is lack of communication. On one hand you've got Okuda's scenic art department doing the MSD and DP, and on the other hand Foundation doing the ship CGI and coming up with their own.

The Bonchune is the nebula-class ship from "Message in A Bottle" (VGR) and the last Ships of the Line calender. The name was not readable in the ep itself, and from the calender it looked like either Bogue or Bodue. But Mojo from foundation imaging (the CGI company that did the calender and the CGI models for the show) confirmed that it said Bonchune, after a colleague of his. There is some doubt as to whether it was actually on the CGI model in the show or if it was added just for the calender, but at least for now I'm including it for complete-ness's sake.
The Spector is the Akira-class ship in "Message in a Bottle" (VGR). The name and number were in the Fact Files.
i'm afraid though, that i'll have to remove the excelsior Kongo from my list.
The Zandura was a four-nacelled ship that was drawn as a possible ship for the battle in FC, but they never made a model and it wasn't actually in the movie.
The Constitution was seen in a display in "Datalore," in nifty refitted configuration, no less.

1. What episode of DS9 was the Grissom mentioned in?
2. Is there anything canonical that says the Shepard is Oberth-class?

I've added the Transport Container NCC-4000 from TWOK, and the Galaxy-class NCC-70564 (Name unknown until we can figure out what it is) from the DS9 Calender.
What episodes of DS9 showed Galaxies docked at the the station, and could any of these be our mystery ship?

I think there's only one left in it's original configuration. Not Refitted. We know there are other Connies still around because there were three at Wolf 359 (one refit and two prototypes) and there were at least three in service during the dominion War. (The sans-engineering-hull model in the DS9 technical manual, and from the same book, the U.S.S. Curry NCC-45617 and the U.S.S. Voyager NCC-73602, the latter two being Excelsior/Constitution-class and Intrepid/Constitution-class vessels, respectively.)
I could buy the Republic from "The Valiant" being a Constitution, but I'm not at all fond of the Republic from "Court Martial" being one. It would mean that yet another chance to diversify TOS Starfleet would be lost. There was never any indication that the Farragut from "Obsession" was a Constitution, either. Or, say, the Yorktown - but at least she was contemporary to the NCC-1701, and a more likely candidate to share her class.

Totally different Galaxy class. 'Dreadnought' was published in 1986 - Before TNG. The Galaxy class here are Starfleet's first ships designed to explore beyond the Milky Way. They're also mentioned in FASA's Star Trek IV sourcebook. I'm not sure which book came first.
Carey underwent a great "conversion" in the novel "Challenger", part of the "New Earth" series. She had already acknowledged Okuda dates for the first book of that series, "Wagon Train to the Stars", but in "Challenger", she quite deliberately began quoting whole chapters from the TNG Tech Manual (the best use of that technobabble I've witnessed so far). It was a rather hilarious experience, actually.
This does not mean that Carey would now be a strict canon-follower - quite to the contrary. She once stated that she deliberately used non-established ship types (let alone names) in "Wagon Train" to emphasize her view that in a realistic (space) navy setup, there are a great many ship designations, far more of them than indicated in canon Trek. I have no reason to assume she would treat the "WYLB" novelization any different - nor the "Broken Bow" novelization she's doing just now.

1. USS Enterprise D - Was it not docked at Deep Space Nine in "Emissary"? If so I think you should add "Emissary" to your list as one of the appearances.
2. USS Enterprise E - When and how was it mentioned in "Life Lines" (VOY)?
3. USS Essex - In the past I remember an Ambassador Class USS Essex being mentioned somewhere. Was it canonical or not? It's just a vague memory I have.
4. USS Hiroshima - This is the first I have heard of this starship being omitted from the Encyclopedia. Do we have an explanation as to why it was omitted, and were any of the CGI models on-screen labelled Hiroshima?
5. USS Lantree - When was it mentioned in Star Trek: The Undiscovered Country?
6. USS Voyager (NCC-73602) - When was this ship mentioned in "A Time to Stand"? If it was given this registry, someone should be shot.
It would be great to get those things clarified.
4. The Hiroshima was in the second encyclopedia, from 1997, at the back of the book in the ship chart. It was replaced by the USS Appalachia in the 1999 edition (also, the Nebula-class USS Phoenix shows up in the third encyc in that ship chart, replacing the USS Sutherland).
6. That name and registry are unofficial, and that class never showed up in an episode (to my knowledge). However, it would not be out of line if Starfleet renamed a ship Voyager if they thought the NCC-74656 had been destroyed.

Anyway, I would have thought in cases such as these a standard investigation may take place for a year, involving a search pattern around the last known coordinates, and tracing outwards from that point for a certain amount of parsecs etc. If nothing is found perhaps, whilst not participating in any active searches it simply remains an open case for a further two years, until then being 'officially declared lost'.
Anyway, i'm too tired to sift through the data, but can someone give me the link to the most respected Starship list (with both Canon, and Non-canon ships ; how you define that is up to you )

-Sol System, yes the political climate at the time would've certainly been a factor. The Cardassians I'm sure were seriously investigated in regards to Voyager's disappearance. But also the fact that it went missing in an extremely volatile environment - the Badlands. Perhaps many suspected that the ship succumbed to the dangerous conditions in this area, and Starfleet probably spent a good while searching for any signs of a wreckage.

1. You're absolutely correct. I'll update.
2. I can't remember specifically. That came from Spike's list, where I got a lot of my episodes from.
3. No. There has been an Ambassador-class Exeter and an Excalibur, but no Essex. Thus was the basisi for a small theory of mine that you can read about in the thread labeled 'Museum Ship.'
4. As far as I know, it was a 'mistake' in the second Encyclopedia, and wasn't in FC, but I'm just including it to be as complete as possible.
5. The Lantree was listed on a display chart on the Enterprise bridge in TUC.
6. This is a somewhat controversial issue. In the Deep Space Nine Technical Manual, there are several ships depicted which are 'kitbashes,' or ships made from bits and pieces of others. Among them are these:
(a.) A Constitution-class Variant.
(b.) An Excelsior-class Variant.
(c.) An Intrepid-class Variant seen in several DS9 episodes including "A Time to Stand," to which the DS9 Art Department refers to as the Yeager-class.
(d.) Another Excelsior-class Variant, seen in DS9 "A Time to Stand" as the U.S.S. Centaur NCC-42043
(e.) An Excelsior/Constitution-class Variant, seen also in "A Time to Stand" as the U.S.S. Curry NCC-45617
(f.) An Intrepid/Constitution Variant.
Now, while only three of these could be clearly seen in the episode "A Time to Stand," Mike Okuda has told inquirerers(sp?) that all of these designs were in fact built as models and filmed for the episode.
It just so happens that that last one, the Intrepid/Constitution variant, bears a striking resemblance to the original Intrepid-class study model built in pre-production for VOYAGER. Pictures of it appeared in The Art of Star Trek:
Some people have theorized that it is likely that it was that study model which was used for background shots in "A Time to Stand," and that it is what the picture in the DS9TM represents, rather than a completely new-built model. I happen to agree with them, and so that is why it is listed as it is in my shiplist.
