So first appearance takes place during the first four seasons of TNG, and it appears in at least five episodes. (Unless Jeff is actually saying it appears in no episodes at all, but I don't see how that's possible.) Unless there are an awful lot of flashbacks to that period I'm not remembering, that means it must appear multiple times in those four seasons. It's not alive, and not independently warp capable. That's a very narrow field.

Does its first appearance take place in an episode of the first two seasons of TNG?

To clarify, it has been seen in more than 5 episodes of TNG, not limited to the first five seasons. The question did not clarify whether it was of those first 5 seasons, or expanding to all Star Trek Episodes, so I'll give this to ya.
